You arrive on the scene and find a
68-year-old male patient whose wife states he suddenly began to slur his words.
The patient is alert and responding to your questions. You note an obvious slur
to his words in addition to a left facial droop. His respirations are 14 per
minute with good chest rise. His radial pulse is irregularly irregular at a
rate of 108 bpm. His skin is warm and dry. His SpO2 is 92% on room air. His
blood pressure is 242/128 mmHg. His blood glucose reading is 89 mg/dL. He has
no known allergies and takes Lipitor. He has no significant medical history and
last ate when he had lunch about 2 hours prior to your arrival. The patient was
on the phone when the slurring suddenly began. He has obvious facial droop when
you ask him to make a smile. His left arm drifts when he attempts to hold both
arms out in front of his body. His pupils are equal and reactive. Breath sounds
are equal and clear bilaterally. Good pulses are found in all the extremities;
however, the left arm and leg exhibit weakness and loss of sensation.
